Which badges can I put in slot four?

Any badge from the $22 to $26 tier: Sports, The Great Outdoors, Hometown Pride, Pet Badges and the Grateful Dead range. The picker only shows badges that qualify, so if you can see it, you can load it.

Will the badge come off the truck?

Not on its own. The 3M automotive-grade adhesive is the same style of bond used for factory trim, so it stays put for as long as you want it to. If you ever want it off, heat and fishing line do the trick without hurting the paint.
